- Passport Health Plan is the operating name for University Health Care, Inc., a Medicaid managed care plan that serves the Medicaid and KCHIP populations in the Commonwealth of Kentucky. The Plan was established in 1997, by a group of providers with a history of caring for Medicaid members. Those providers include University of Louisville Hospital, Jewish Hospital, Norton Healthcare, and the Louisville/Jefferson County Primary Care Association, which includes Jefferson County Health Department and Louisville's two federally qualified health centers, Family Health Centers and Park DuValle.
